A personalized care plan is a written plan that outlines the physical, social, and emotional care tasks required to meet a person’s changing health needs.

  • A personal care plan is a document that lists a person’s specific needs, preferences, and goals for their care and well-being. It should be created collaboratively between the person needing the care and their health care providers, family members, caregivers, and care partners.
  • Know that you can have a personalized care plan if you are living at home, in hospital, or in assisted living or residential care. It can also be part of your palliative care or hospice care.
  • Start by assessing your ability to perform daily activities independently, such as bathing, dressing, grooming, eating, and mobility. Any assistance or support needed for these activities should be written down. Include any specifics about how you want these tasks completed.
  • Learning what a person needs for support can also be identified by a health care professional coming to your home and providing a Home Care Assessment.
  • Make a complete list of the areas you need help and support with, the time of day, and your preferences on how the tasks are completed. Some topics and care needs should be considered through all tasks including mobility, safety, and emotional support. Think about your needs with personal care, medication management, emergency response, meal support, active living, social connections, and emotional health.
  • A written personal care plan is important in ensuring your family caregivers, care partners, and hired caregivers understand your needs, wishes, and preferences.
  • Review and update your care plan regularly to ensure it continues to meet your needs.
  • Encourage family caregivers to learn how to provide personal care safely from Healthlink BC.
  • Complete the activity Changes in My Personal Needs and Coordinating Home Care Services in the section My Place to Call Home in your Mywell Health planner to help identify areas where support and care are needed.
