A health profile is a personal summary that should include your medical, surgical, mental, and family health history, medications, allergies, and vaccination records.

- Complete your health profile with current information about your health conditions, medications, allergies and sensitivities, infectious diseases, family health history and surgical history, vaccination records and emergency contacts.
- Learn how to use your Mywell Health planner to organize your health information. List all your health conditions in your health profile and include any handouts and print resources you have collected from research-based websites like Healthlink BC.
- Understand that your personal health profile is something that you can create and maintain, but it is not the same as your official medical health record (MHR) or electronic health record (EHR). These health records are owned by your physician, hospital, or health authority.
- Review Healthlink BC’s list for organizing your health information.
- Learn about online health information portals such as Health Gateway BC.
- Print a physical copy of your updated health profile for your Mywell health planner binder and share a digital copy with your health advocate, physician, nurse practitioner and those who are involved in your healthcare.
