Get started on your Mywell Health & Lifestyle planning for Seniors. Created for people 65 years and older – for yourself, your partner or a senior you are supporting. The Mywell Health activites provide guiding questions, discussion points, and current resources to help you build your health and lifestyle plan.
How do I get started on your Mywell Health planning for Seniors?
Choose any section from below that interests you. A good place to start is My Health Information, which will help you identify and organize your health information. Each person should have their own planner. You can complete your own planner or help create one for someone you are supporting.
What is the Resource Kit?
Each term that is underlined in your planner will link you to the Resource Kit for definitions, tips, tools, links and videos to support your learning.
Make a print health planner
While we live in a world of smart phones and computers, some people might like to have a print copy of their health planner.
- Print each PDF activity or section as it is completed or blank for review.
- Start a Mywell Health binder and use divider labels for each of the (6) sections (My Health Information, My People, etc) to organize your work.
- Add your reports, information, and notes into each of these sections.
- Review and update as needed.
Share your health planner online or via phone
Download your PDF activities or sections to your computer and save them to a folder (Your Name) Mywell Health planner.
Set up (6) folders labeled (My Health Information, My People, etc) for each of the sections.
Add your digitial reports, information, and notes into each of these sections.
Save all your files to a cloud-based file system to share with those you trust or for access online or via phone.
What about tools for Adults, Youth, and Children & Families?
