How do I use the Mywell Health planner?
Can anyone use Mywell Health?
Yes, Mywell Health is for people living with health changes or challenges as well as those supporting them. However, people not currently experiencing health challenges are also using Mywell Health to outline their health and lifestyle choices and communicate their needs to their care partners and those they trust.
How do I use Mywell Health?
Mywell Health has a number of topic sections with activities for you to complete. Each topic has activities to help you learn where to find information to make informed choices.
Mywell Health Inc. does not store your information or data online. We have created downloadable and fillable PDFs that you can save to your electronic device. To edit the PDFs you will need to download and save them on to your preferred electronic device (computer, tablet, smart phone).
Work in the downloaded PDF and start by inserting your name and the date so it appears on each page. Complete the activities by filling in the information suggested. Save your data to your computer after every use. We recommend that you save your folders to a cloud storage (Dropbox, Google Drive, ect). This will ensure your files are backed up and easily accessible from any device (Computer, Table, Phone).
What is in the Mywell Health Resource Kit?
Entering a new phase in life or health can be challenging. Sometimes the language is new and unfamiliar, and navigating the who, what, when, and how can be overwhelming. The activities in Mywell Health are designed to assist you in learning about new terms, options, and services. The Resource Kit supports your learning by offering definitions, access to research-based websites, educational materials, videos, and other resources to enhance your knowledge and empower you to make informed decisions.
How can I Share my Mywell Health planner information with others by computer or phone?
Download the Mywell PDFs activities to your computer and save them to a master folder with (Your Name) Mywell Health. Within this folder, set up secondary folders using the Mywell topic headings such as (My Health Information, My People, etc).
Save each of the activities completed into the appropraite folder. Add your digitial reports, information, and notes into each of these sections.
Save all your files/folder to a cloud-based file system (Google, Dropbox, other) to share with those you trust or for access online or by phone.
Can professionals use Mywell Health and the tools in their practice?
Absolutely. Everyone has access to mywellhealth.info and the information it provides. If you choose to use it in your professional practice, please review the Terms of Use & Disclaimers thoroughly. Our platform offers thought-provoking questions and tools to encourage discussion and information gathering. We do not provide specific guidance or advice. By encouraging health literacy, one of our goals is to enhance accessibility to research-based information that is already available. The Mywell Health planner and Resource Kit can be used as a supplement to your teaching and practice.
Does Mywell Health offer health care advice?
Definitely NOT. Mywell Health provides thought-provoking questions, activities for recording your information and wishes, and resources to assist you in understanding your options. For more information on this, please review our ‘Terms of Use & Disclaimers’ section.
Can Mywell Health help me organize my mom’s health information and her daily care needs?
Yes. The first section of the planner, ‘My Health Information,’ will guide you through the process of collecting and sorting her health information. Proceed through the next sections, ‘My People’, ‘My Day-to-Day’ and possibly ‘My Place to Call Home’, to help you identify her needs and assess her existing resources.
My husband and I are starting to use Mywell Health for our health and future planning. Any suggestions?
Start a Mywell Health binder and digital files for each of you. Be sure to add your name to the activities for safety and accuracy. Many of the activities can be completed together and with your family members or those you trust. Keep a current digital file folder for each of you. Add decision documents as outlined in the activities and any other pertinent information.
How can I make a Mywell Health Planning binder?
While we live in a world of smart phones and computers, some people might like to have a print copy of their health planner.
- Print each PDF activity as it is completed or blank for review.
- Start a Mywell Health binder and using divider labels for each of the topic areas such as (My Health Information, My People, etc) to organize your work.
- Add your reports, information, and notes into each of these sections.
- Review and update as needed.
Cost
How much does it cost?
There is ‘no cost’ to using the products in Mywell Health. You can download the Mywell Health planners and access the Resource Kit for free at any time. The website is funded by private donations and grants with a goal to improve health literacy and awareness. We operate as a social purpose project.

Privacy & Confidentiality
How does Mywell Health keep my information private and confidential?
Mywell Health does not collect or store any personal information. You simply download the Mywell Health planners to your personal computer. You fill in the activities and save the information to your personal computer files. We do not provide data storage thereby, ensuring that we never have access to your personal information.
Can I share my planner with my family?
Yes, that is an option. Mywell Health was designed to encourage communication among you, your family, and your care partners. After working on or completing your activities, you need to save them to your personal computer. If you choose, you can share your files with others by adding them to a cloud-based storage system or sharing them in print or by email.
Can I see my planner information on my phone?
Yes. If you save your planner into a cloud-based storage system and have the app on your phone, you will be able to access your information at home, during appointments, or in an emergency. However, it is recommended that you complete the activities and data input using your personal computer.
Getting Started
I am not sure where to start?
By exploring Mywell Health, you have taken the first step in focusing on your health and lifestyle planning. Download the activities or sections that interest you. Personalize each section by inserting your name and date. There is no set order to complete the activities or sections – it is dependent on your situation and need. Many find that starting with ‘My Health Information’ is often the easiest place to begin.
How do I start talking to my parents about these important topics?
Placing your parents in the centre of the conversation is the most important step. Create a non-threatening and calm setting to begin. Use the planner activities to guide the conversations. Listen attentively and write down their stories, capturing who they are and what they want in their future. From there, they can choose what topics they would like to explore. Ensure you create a planner for both your parents by inserting their individual names and revision dates into each section. This is especially important for ‘My Health Information’. Provide each of them with a print and digital copy of their planner for safety and convenience.
Can I use the Mywell Health planner for my partner who has developed dementia?
Absolutely. Mywell Health was designed for people facing health challenges or changes. Choose the Senior planner and start by completing the first section called ‘My Health Information’. Each section will guide you through several activities that encourage discussion and exploration. Learn about available options for services and support in the Resource Kit.
What if a person is not very comfortable using the computer – can they still use Mywell Health?
Yes. You will need someone who knows how to download and print off the pages. The Mywell Health planner was also designed so that it could be printed, written in, and stored in a binder. In fact, people who are comfortable on the computer also keep a printed copy for easy access and a place to add documents and other information.
